The Community Assessment Program

The Community Assessment Program (CAP) strives to keep youth at home and out of juvenile justice and human services systems by connecting families with supportive services to promote safe, healthy and happy kids. Young people receive early intervention screening and assessment to identify factors contributing to concerning behavior and factors mitigating risks. Referrals are accepted from parents and professionals. 

CAP’s Comprehensive Assessment includes: screening for immediate medical needs; behavioral health screening (including mental health and substance use); assessment of risks, strengths and needs; case planning and short-term case management.

General Referral   Parents and caregivers as well as professionals may request to set up an appointment with a behavioral health specialist. Referrals for assessment can be taken over the phone of by completing the General Referral Form (Coming Soon) – Please Call  or complete the Contact Us section to the right of your screen. 

Aurora’s Gang Reduction Impact Program  The JAC A-GRIP Bilingual Mobile Assessment Specialist provides assessment services locally in original Aurora to prevent youth from becoming involved with gangs. Click Here | AGRIP Referrals

School Services  Any school within Arapahoe, Douglas, Lincoln and Elbert counties may refer students to CAP.  Aurora Public Schools, Cherry Creek School District, Englewood Schools and Littleton Public Schools receive specialized services from CAP as they are funding partners. Click Here | Information on the School Referral Program

Court Referrals  Local Magistrates and Judges may refer young people for assessment if it is determined supportive services and/or positive strategies may be helpful. Click Here | Court Referrals

Rural Intervention Specialized Case Management JAC partners with the 18th Judicial District Senate Bill 94 Program, Rural Integrated Service Engagement (RISE), and Connections for Families to provide the Rural Intervention Specialized Case Management program (RISC) in Elbert and Lincoln counties.
